CBDPP Home - Beryllium Survey - FAQ

Theta Pro2Serve Management Company, LLC (TPMC) is partnering with the U.S. Department of Energy (DOE) and other DOE sites to create a registry of those former and current DOE employees, their contractors’ employees, and their subcontractors’ employees who may have been exposed or potentially exposed to beryllium in the workplace at any DOE facility.

The purpose of the program is to find those who may have symptoms related to beryllium exposure and to propose surveillance and/or treatment of those symptoms. In the long run, the data gathered may help health professionals better understand what causes these symptoms and at what level or concentration. With such information, we can better manage workplace exposures to reduce, if not eliminate, the development of symptoms in others.

As a precursor to inclusion in this registry, a survey is being offered to all TPMC employees to determine whether or not they have knowledge of real or potential exposure to beryllium during their work history with DOE, its contractors, or its subcontractors.

Please read the questions on the linked beryllium survey and answer them to the best of your knowledge. If any question is not applicable (NA), just mark the question NA. The survey then should be sent to:

The Southern Ohio Medical Center medical staff there will collect the responses and assign a unique identification number so that any response can not be traced back to the individual. The responses will then be used to determine to whom an invitation will be given to participate in the beryllium monitoring program known as the Chronic Beryllium Disease Prevention Program (CBDPP).
